ceftriaxone is first line for pyelonephritis. it is part of the current guidelines because there is too much resistance against cipro these days. ampicillin and genta are not good choices cuz gentamicin is harmful to the kidneys.

yea personally in my IM rotations most pts were on IV ceftriaxone until sens and cultures came back......

Gram negative organisms= ceftriaxine or flouraquinalones
Ampicillin n gentamicin not used buz of inc resistance.
In Pregnancy also cephalosporins.
