MBME 2 Q.
A 45 year old woman has 3 month history of a persistent cough & increasing shortness of breath. She denies chest pain or other illnesses. Blood pressure is 110/70mmHg, and pulse is 110/min. she has diffuse basilar crackles & an S3 gallop, but no murmur. ECG shows sinus tachycardia & nonspecific ST-T wave changes. Echocardiography shows a dilated left ventricular & a diffusely hypokinetic heart. The most likely diagnosis is
