NBME 7 BLOCK 3 Questions - mdorbust001

1.) 37 year old man comes to the physician bc of persistent numbs in his hands and feet for 10 months & weakness in his left wrist for 3 weeks. He renovates old houses. During the past year, he has made several visits to the ED bc of abdominal pain and was subsequently diagnosed with gastritis. BP 135/95. Mucous membranes are pale. Abdo exam shows no abnormalities. Neuro shows weakness w/dorsiflexion of left wrist & loss of sensation in a stocking-glove distribution bilaterally. Hct 32%. BUN 35mg/dL. PBS shows microcytic, hypo chromic erythrocytes. 2.) Five days after undergoing an open splenectomy for ITP, a 57 year old woman has the onset of SOB. During the operation, dissection of the splenic hilum was difficult. Only med is morphone. Temp is 99.2F, HR 80, RR 20, BP 120/80. Surgical wound appears normal. Decreased breath sounds at left lung base. Leukocytes 15,600/mm3, platelet count 120,000. Amylase is 90U/L. What is next step?
