a 65 year old man comes to the emergency department 30 min after an episode where he could not speak and had transient blindness of the left aye. he is right handed. he has type 2 DM and has been treated for hypercholesterolemia. Neurologic exam shows no abnormalities. No evidence of residual speech deficit, and visual fields are intect. No motor deficit. Fundoscopic shows a highly refractile bright yellow plaque on the left.
