HERE IS A TYPICAL STYLE QUESTION FROM FORM 7, but it is NOT from Form 7:

32 year old male presents to a new physician with complaints of lack of energy. He lost his mother from lung cancer 2 years ago. It's coming up on her birthday and the holidays and it has been very difficult for him for the last 3weeks. During this time period he has called in to work 5 times, something he rarely does. He doesn't participate in Saturday night card games with his buddies anymore. He complains that he only gets a few hours of sleep each night and feels excessively tired during the day. In addition to his complaints, he is also concerned about his chances of getting lung cancer like his mother, being that he also smokes. He has a 20 year pack history. Aside from counseling the patient on his concerns, what is the next step in management?

A. Get a baseline BMP
B. Prescribe a selective serotonin reuptake inhibitor
C. Prescribe a typical antidepressant, such as MAOI
D. Assess the patient for a history of any seizures.
E. Order a chest xray.

B. due to major depression?

Your reasoning is correct, but he is also a smoker who is concerned about complications of such....

So what Rx would you start him on?

B. You start him on SSRIs. The pt was already counseled on Lung Cancer and most likely was advised to quit smoking, so what is left is to treat his depression

We have to start him on Bupropion which helps for both depression and smoking cessation. So ask for a history of seizures because bupropion can cause seizures

CORRECT ilovmedicine!!! Wellbutrin/bupropion is the drug of choice, but it is important to know if this patient has any history of seizures.

We often assume that these questions have been covered in the interview, but the exam writers want to know if we know the significance behind select interview questions as they relate to the treatment or diagnosis in mind.

Typical Form 7 questions. Long gone are the days of answering questions based on associated information. We've got to really know the concepts thoroughly. I am having to go back through my notes much more thoroughly.
