Acetaminophen toxicity - gogock

MZB 3 =pg 493 says NAC to any pt with possible overdoes and don't wait till the serum level result comes .

but Uw q #3136 pt with unknown [pt said 14tab] dose comes wait till 4th hour after intoxication to even determine the serum acetaminophen level and immediate NAC not the answer

how do u see it ?

According to Harrison's

In a pt with high acetaminophen blood levels (more than 200microgm/ml AT 4 HRS or more than 100microgm/ml AT HRS AFTER INGESTION) administration of NAC reduces the severity of hepatic necrosis.

So, determination of secum acetaminophen level does looks the right choice before starting NAC.
