A 46 year old man comes to the physcian because of an itchy rash that appeared in his groin over the previous several weeks and seems to be spreading. He has tried using over the counter itch creams but it has not been effective.His medical history is significant for diabetes uncontrolled with diet, and hypertension for which he takes losartan. He has no known allergies to medications and the family history is negative for skin diseases and Cancer.He does not drink or smoke. On inspection, there are sharply demarcated, dry,brown, slightly scaly patches in both the genitocrural creases.there are similar but smaller patches in the axillae and in the fourth interdigital space on the feet. A woods light examination is postive in all the three regions and give a coral red colour. What is the next step in management


A. Doxycycline 250mg four times a day for 2 weeks
B. Topical steroids for foru weeks
C. Erthromycin for one week
D. Fusidic acid topically applied for two weeks
E. Topical azoles for 8 weeks, two times a day

The skin condition is erythrasma, caused by Corynebacteria , it is treated with systemic erythromycin or topical fusidic acid.
