round window rupture does not affect balance only auditory acuity. Round window rupture can heal itself so not all ruptures result in loss of hearing.

Round window keeps fluids in ear contained, the very same fluid that needs to be contained to move around inner ear to affect the hair cells and thus allow for hearing. Once round window is ruptured or fixed, hearing is loss beings. This can surgical be repaired to restore hearing is some cases.

Vestibular neuritis and labyrinthis are disease of infection in ear which affects the vestibulochochlear nerve, needed to send signals for balance and head position.

Clearly the later disease is more likely to cause spins.
