You rank X number of programs, the rank order does not affect your chances so the rank order is up to you.

By computer algorithms, but really the program must rank you and you are in line with others to get the spot at that residency and in a way its simple

You rank the program as 10th
The program ranks you as 21th
They have 30 spots, you already have one unless you ranked a program 9th and you are a match to them as any number.

Now what if they are the only program and they ranked you 13th and they have 12 spots? As long as one of their other pics (1 through 12) has matched elsewhere, you will move up into a spot, if not then you will not match there.

You see your picks only matter if the program picks you too

The program decides who will match there
You decide only if you have more than one program who would match you.

Its simple but put 40k people into the system and its complicated
