If Metronidazole does not work, do we give Vancomycin IV or Oral????

ok hsar thx forthe question
first of all u have to discontinue the causative antibiotic and give IV fluids and correct electrolytes ,then if this didn`t work or u can`t discontinue the AB then u add oral metronidazole which is preferable on Vancomycine(ORALLY).
Mertranidazole if didn`t work then u will go to oral vancomycine
look :
metronidazole is preferable because:
less expensive and minimize the emergance of vancomycine resisitant Enterococcus.

Yes, thank you! So we give oral vancomycin instead of IV because IV vancomycin does not cross the bowel wall but Oral does.

and to hsaar or anyy body ; what is the side effet of metronidazole and what is the side effect of vancomycin/?

why are U so much worried ABOUT VRE?

Metronidazole: Common S/E N, V, D, dyspepsia, metallic taste. Disulfiram-like rxn with alcohol, Seizures & Peripheral neuropathy

Vancomycin: Nephrotoxicity, Ototoxicity, Thrombophlebitis & flushing ("red man/red neck syndrome, which can be prevented by pretreatment with antihistamines & slow infusion rate)
