Do fellowship programs support H1b visa or it's just J1?

There are plenty of programs supporting fellowship training under H1b visa.

Depends on how a specific program feels about having the convenience of an applicant processing his/her own J1 visa with the ECFMG vs sponsoring an H1B visa and finding the immigration lawyer and maybe, maybe paying the fees associated with it.
Also, J1 applicants may have the opportunityof applying for a 4th year of fellowship after the usual 3 years if choosing cardio, GI or pulm/CC (for interventional, sleep, etc).
