A 14 y/o M presents to you with a deep skin abrasion on his knee. He states that it has not stopped bleeding since it happened during recess ~ 20/30 min ago. On PE you note well developed/nourished adolescent. There are multiple purpura over his legs and arms, and a few scattered petechiae on his chest & gums.
Pat
Hgb: 11g/dL
BT: 22 mins
Platelets: 290,000/mm3
A trial of cryoprecipitate transfusion does not improve pat BT. A normal platelet transfusion does improve BT.

Dx ?

a) BS syndrome
b) HSP
c) ITP
d) TTP
e) vW disease

Hello @cardio69,
thanks a lot for the explanation. On this I'ld say AA.
C/D ruled out with the normal platelet count, vW ruled out with no improvement with cryo, and in HSP, i'ld expect some abd pain, arthralgia. Please correct me if I'm wrong
