@paytoll, i didnt do any course, just went and asked the physician if i can work there, told her about me being IMG and steps, i do basic stuff like billing, phone calls, taking history/vitals, tests like strep test, some blood work, doing physicals, u dont need a course for this stuff,

I agree with @paytoll that IM is definitely a better option than FM. FM is very competitive.
