It is my destiny i think but what is its place in USA i mean job opportunity ? next is there any combined programm of psych and int med? what does combined programm mean ?

There are combined Family Practice/Psychiatry programs. I also know that there is a very big need in psychiatrists at least in the Midwest. Patients sometimes wait for their appointments for 3-4 months. So, do an additional search about this specialty and then decide by yourself.
