Is A-a gradient calculation needed in Step 3??

Calculation itself is not accurately needed but we need to know basic quick calculation to determine whether a respiratory failure is predominantly hypoxemic or hypecapnic or mixed.
Archer lectures on Nephrology and Pulmonology presents these topics in a very simple way.

There are Qs sometimes on etiology based on blood as analysis and next steps there. A-a gradient basics can be very useful there.
