Answer is D.

ESRD with GFR unable to excrete phosphate and reabsorb ca. But there is a deficiency of vit D (active form) which helps absorb ca from gut--> low calcium levels,Although there will be secondary hyperPTHism to this low calcium level but will be never enough to raise serum calcium levels, hence despite of secondary hyperPTH, there will be low calcium levels seen.
