NBME 16 block 2 question 14 - futuredoc2022

Did anyone figure out what microganism was shown in the question? The answer was amphoceterin B. I was between this and the carbapenem but I couldn't figure out if this was a funal or bacterial infection.

There was an old thread about this question but I still wasn't convinced. I'd appreciate the help Smile

one more thing

are there any PAS+ bacteria? if not then thats the clue for chosing amphoceterin B over the carbapenem.

I think it's Blastomycosis, if I am right then Amphotericin is the choice
