hi friends,
there is a case of MI in ccs section of usmle123.
in that case they have not given thrombolytics but given heparin eventhough pt. presented with in 12 hr. period?
i dont understand why?
please give ur input who has subscribed usmle123

I think the distinction in that case is between thrombolytics and angio, not thrombolytics and heparin. They chose to use PTCI instead of thrombolytics, but you could probably also give thrombolytics rather than doing an angiogram. I think the heparin is just to buy time until angio can be done in that case.

123 software does't give the option of thrombolytics. During the case they poorly explained that generally PTCI are preferred over thromb. I think having the same case and both options on real exam both answers would be right.
