any penetrating wound----------laprotomy---exception single gun shot wound.
with blunt trauma, pt. unstable-------do dpl , although usg is preferable.

blunt trauma,if pt un stable do DPL or U/S if stable do ct scan.

Pt in hypovol shock ,has signs of peritoneal irritation........ go straight to Explo Lap

Pt stable but suspected int abd bleed, do CT scan

Pt in hypovolaemic shock , no obvious signs of peritoneal irritation( after excluding pelvic fracture and fracture femur) ,to confirm abd bleed do DPL or USG

Whenver the patient is unstable and you can't diagnose by ultrasound. All stab wounds to the abdomen with obvious penetration of the peritoneum needs exploratory laparotmy. Look for signs of peritoneal irritation/shock.
