Yes, you have to put US address and US phone no., it is mandatory.
The contact information msg is permenant, dont worry about it, it will remain even after your application is approved.
"Request IAP" is a good sign, it means that "hopefully" everything is O.K, so just wait, BTW, IAP-66 is the old name of the DS-2019 so they are the same.
It took 7 days for ECFMG to issue my DS-2019 form.

Yes, I agree with you, it is rediculous to put US address while u r outside USA. Try to remeber any residential address. I dont know if they can issue DS-2019 without that.

hello all
i just have one question. how do u access your J1 visa application status on oasis. I have paid online and sent my form to my program, how will I be able to see my status. I was a little confused as I dont see any link about J1 status.

Once they receive your application, you will this link. Usu the TPL at your program, once they fill section B of the application and review your whole package they send it to ECFMG via courier service so just wait few days and check with your program if the have sent it to ECFMG or not yet..

Hi! I just wanna ask what US Address to put in OASIS? Is it advisable to use the address of the hospital where you matched? Or just use a friend's or a relative's address residing in the U.S.? This doesn't make any sense 'coz ECFMG is gonna mail the DS-2019 to the TPL anyway so I don't understand the need for a U.S. address in the application/OASIS (?)
Any inputs guys? Any response response? Thanks in advance!

You cant put the address of your program, it must be a (residential) address, put the address of one of your friends. I agree it is non-sense but u have to do it..GL
