ok guys i think we should come to the conclusion of this question.

Correct answer is AA.The clinical scenario describes a patient withtype 2 DM.The diagnosis is strongly suggested by the clinical presentation and the presence of glucosuria.but confirmed with an abnormal 2-hr postparandial serum glucose(upper limit of normal is 200mg/dl).In type2 DM there is hyperglycemia due to insulin resistance and therefore hyperinsulinemia.In this case there is activation of pyruvate dehydrogenase in the liver.Pyruvate is convertaed to acetyl coA.therefore,one would expect increase levels of acetyl coA (option D).

thanks sunshine! is it true that hyperinsulinemiain is present in all type 2 DM (maybe early)? i am curious because sulfonylureas are commonly used in treating type2 DM and they do so mainly by increasing the insulin level.
