its true that a painless mass is taken to be scrotal tumor unless proved otherwise-but here it is tender....
what book have u taken this ques?

Thia was the explaination given:

Epididymitis is unlikely in this patient given the sexual history, as well as the physical examination.
Patients may often present with these symptoms instead of the pathognomonic painless testicular mass.
