How do we know which type of CXR to pick on CCS?

yea, so when do we pick which CXR based on common patient scenarios on CCS?

always portable in the ER
PA and lateral in office

Simple reason is that ER patient is too unstable to be transported to radiology dept for pa and lateral

agree with misshyd just to add one thing ER we have limited time so portable becos results of portable CXR come back in 30 mins.

Thank you, but don't we need PA and Lateral in ER if its someone with a perforated ulcer?

perforated peptic ulcer is a different issue. You can do a portable cxr here but should be done in upright position.
An upright portable CXR or Plain abdomen x-ray in left lateral decubitus position are very good to see for air under diaphragm and pneumoperitoneum respectively.
