"If you can find the OB or externship by yourself, of course you do not need to spend so much money. But the reality: It is so hard to find such a chance in US. "
---- nothing can be far from the truth. Just mass mail some programs and respectfully ask them for a research/observership opportunity. Atleast someone will always respond and you can take it from there - has worked in so many cases. You end up with solid Univ experience on your application with taht!

THE Total IMG's Experience Guide
PUT
April 16, 2009 Imgs USMLE
into www.google.com search and it is the highest Blogspot (top 1 or 2) result
i.e.,LOOK ONLY FOR THE #2, 3 or 4 SITE- the one that is listed as a BLOGSPOT address- with more than 26,000 views
#1 MOST IMPORTANT PRIORITY BEFORE YOU EVEN BEGIN!
first finish step 3!-(get greater than 85 with no attempts; i.e., ALL IMG Friendly programs/externship directors- they have spoken to me personally and discuss this at the annual residency directors' meetings- NOW REQUIRE THIS PASSAGE AS A REQUIREMENT EVEN THOUGH THEY MAY NOT STATE THIS SPECIFICALLY; essentially -especially in the IMG friendly programs- too many low step 1 and step 2 scoring residents are unable to pass step 3. in most programs now they will not even let the resident graduate past PGY-2 without step 3 passage. then this failing resident becomes a liability for the program director and creates difficulties in filling the residency spots; also this person cannot even get a medical license so that he or she cannot serve to moonlight in the hospital ER at times of house staff shortage )
SEE THE RATINGS at that link for information on the various USMLE courses
Also look at these !

I am so glad to share my opinions with IMGs who are looking for the American experience in the USA Health Care System.
Frankly speaking, I am happy with AMERICLERKSHIP company.
Nobody recommended them to me. I did my research and suddenly found the company which was ready to provide me an opportunity to actively observe the clinical fields I was very interested in.
I am not fresh on the market of ERAS , but observations I tried before , the LORs ,
the statements I wrote, - were not even close to all activities I really experienced , all rules and regulations I really went through, all my professional weakness I was really able to see and admit after I accomplished AMERICLERKSHIP 20 weeks of clinical rotations.. I am OB/GYN doctor. My goal was to prepare myself as good as enough to be professionally confident and ready for real Physician job . I wanted to know everything about my preferable medical field.
I must put myself in the real doctor's life, including regime, physical and emotional pressure, ability to be a very fast learner with flexible adaptive personal skills, ability to listen, observe, remember, summarize, appropriately report all above, and then , when finally come home be able to let yourself know " Listen, body, YOU are marvelous, YOU are genius, YOU are so good Doctor! You were very productive, strong. Now , just open the book or go online and just be ready again to study about all cases YOU were so lucky to see today. "
With all my background, previous experience , USA work, observations I never experienced so much stress being at the new place with new rules . WHY? Because those clinical rotations were absolutely independent. New places, hospitals, clinics, private offices, doctors, attending physicians, nurses, all staff on one side and I am along with my IMG experience, personal " English structure", and family problems on the other side .
Normally, nobody would ever plan to have clinical observation in the different state, far away from home, pay tolls , spend about 3-4 hours in the car. Nobody would agree to participate in such difficulties . I was placed in such condition for only 8 weeks to observe the OB/GYN Doctor who was real " GOD " in his specialty.
I got learned not only the professional tools. I tried to observe and accept his way of thinking, manners, personal skills, interests, life style . It was very important for me to understand what makes this doctor to be different , what helps him to overcome all the outstanding pressure. I followed him everywhere including Inpatient, OR,Outpatient units, observing all invasive, noninvasive procedures of complete OB/GYN services, listening the presentations at the morning rounds. It was very clear that I put a lot of effort to see everything what this Doctor is good at .
I am proud of myself that I did it very good. The next rotation was provided at the rural area, about one hour by car from home.
Family Medicine clinical block I started with different understandings of what my goal is about, what I must see, what kind of weak points relating to FM need to be improved . So, I started to observe the Doctor with 27 years of experience in General Family Medicine. Nothing bothered me. I was comfortable and appreciative with urban ,rural region. My intelligence and professional manners were on the top and equal regardless the low income , insurance, illness, complaints. I had observed the work of the Doctor who is "Everything ' for his patients. As a physician, he showed the classical work of General Family Doctor who knows the patients problems , who really cares and doing his maximum for the best of his patients.
At the end of each rotation I asked for LOR . My personal impression of honestly written LOR increased the respect and dignity to myself .
Not everybody could go through the walls, overcome all the difficulties , learn and speak new language, understand and accept new system. It was my test , it was my main question to myself :
"How strong I am as a person , how good I am as a doctor ? What I am made of? If I am able to face and handle all this unpredictable difficulties, resolve them in time, learn on the mistakes, normally deal with unknown situations and not be shy asking for help, continue to be emotionally stable , wise in priorities and conclusions , - then I could have an answer to the questions and always help myself . If I found the way how to help
myself , I will do everything to help my patients.
I am glad that AMERICLERKSHIP gave me this opportunity to discover myself during those clinical rotations.
It is known that to get good education is costly. All the service provided by AMERICLERKSHIP deserves the money were payed for . This is including the preparation to the clinical rotations at the hospitals, correction job of the personal statements, resume, CV, given 5 certificates which must be received in advance of the clerkship , verification of the credentials, education, criminal background, requested physical exams with immunization, very organized placement to the hospital or medical center , agreement with experienced physicians , conferences, life webinars , consultations .
After my first call and left message to AMERICLERKSHIP company I was given a call back in the same day. Then , I was interviewed by Dr.P.Mizani for about 40 minutes.
Since that day I was attended by very polite , professional staff. Each step of my preparation to the clinical blocks was accompanied by different responsible coordinator. All my clinical preferences were counted according to my educational background,
IMG practical experience , professional gap . Even I live in the another state , all period of time I feel connected to the team of Americlerkship.
I highly recommend Dr. P.MIZANI with AMERICLERKSHIP professional services . This clerkships are build according to the requirements of the state boards. The paperwork is built based on the ERAS, ECFMG rules and regulations.
I believe I will find my program easily , demonstrate an excellent clinical and personal skills after unforgettable 20 weeks of real AMERICAN EXPERIENCE.
AMERICLERKSHIP is strongly recommended to all IMG who recently came to UNITED STATES of AMERICA and are not familiar with USA culture, healthcare system.
This is real , independent , true AMERICAN EXPERIENCE.
