Acute venous thromboembolism and metastatic ca - harry206

58 yr old women, diagnosed with uterine ca 2 yr ago.
now c/o: b/l pulmonary emboli and mets to liver.
past h/o: 2 yr ago had lower exterimity DVT, underwent surgery follwed by 6 month warfarin.

now pt started on 7 day course of LMWH.

What should be done to prevent recurrent venous thromboembolism in this pt?

(a) transition to warfarin with INR 2-3
(b) transition to warfarin with INR 3-4
© continue LMWH
(d) IVC filter

harry was the pt on wargarin when he devlop pulmonary emboli?

no,

pt developed lower exterimity DVT 2 yrs ago --> at that time she was started on warfarin for 6 months.

2 yrs later, now she preseted with b/l pulmonary emboli and mets to liver.

she is not on warfarin now.

is it true that in pt with h/o malignancy lmwh is better than warfarin?

yes, ans is C.

Chronic LMWH at therapeutic doses reduces the risk for thrombotic recurrence compared with standard-intensity warfarin in patients with venous thromboembolism and cancer.
